Reflection is an amulet in Dragon Age: Origins.
Acquisition
This is given to The Warden by a spirit in the form of someone from their Origin story during A Test of Faith quest in The Gauntlet on the Mountain Top during The Urn of Sacred Ashes main quest. To receive this amulet, the Warden must have answered either "Yes" or "No" when asked if they believe they failed the person(s) the Guardian talks about from their origin story.
