Lost to the Memories is a side quest in Dragon Age: Origins.

Walkthrough

To trigger the quest, approach Orta in the Orzammar Shaperate and ask her what she's doing. She will explain how she believes her family is descended from the extinct House Ortan which was nobility, and is trying to find the proof so that she may present it to the Shaperate and elevate her caste as well as re-establish the ancient House. She will ask the Warden to enter the Deep Roads and find the missing documents.

In order to complete the quest, you must progress far enough in A Paragon of Her Kind for the Deep Roads to have been unlocked. The documents themselves are found in Ortan Thaig, in the large cavernous area with the two bridges (where you will meet Ruck). Retrieve the documents and return them to Orta in the Shaperate. She will ask you to meet her at the Chamber of the Assembly. Do so and talk to her to complete the quest.

Rewards

If A Paragon of Her Kind quest line is not completed, you can ask House Ortan to support either Bhelen Aeducan or Pyral Harrowmont for the throne. Doing so, it will indeed increase the candidate's support among the deshyrs. For more information see here.
If you ask her for any money she could spare you receive 5.

Lost to the Memories - Dragon Age Keep — The quest choices in *Dragon Age Keep*

Whether you ask for a reward or not, you receive:

250 XP

After the A Paragon of Her Kind quest is completed, if you leave Orzammar, return and talk to her again, you will receive 10 and 250 XP.

Note: This does not work if you just go to Party Camp and back.
