Keiran is the son of Morrigan and grandson of Flemeth, born with the soul of the slain Old God, Urthemiel.

If Keiran's father is The Warden:
Keiran was conceived on the eve of the Battle of Denerim as part of a ritual his mother had proposed. In fathering the child, The Warden ensured that the soul of the Archdemon would pass into Keiran, making the sacrifice of a Grey Warden's life unnecessary.
If Keiran's father is Loghain:
Just hours after being conscripted into the Grey Wardens, Loghain Mac Tir reluctantly underwent a dark ritual with Morrigan at The Warden's behest. Loghain conceived Keiran with Morrigan and thus ensured that no Warden would die when the Archdemon was slain.
If Alistair is Keiran's father:
Despite his shock and disgust at the prospect, Alistair eventually agreed to lay with Morrigan, conceiving Keiran and saving the life of the Warden who would have otherwise given their life to end the Blight.
Upon the death of Urthemiel atop Fort Drakon, the Archdemon's uncorrupted soul passed into Keiran.
