Consoling Words is a companion quest obtained automatically at the start of Act II only if you didn't take Bethany or Carver with you to the Deep Roads.
Walkthrough
Visit Merril's Home and speak to her. She will ask about Bethany (or Carver), and tell Hawke not to blame himself.
